{"id":2154,"date":"2025-11-17T16:42:28","date_gmt":"2025-11-17T16:42:28","guid":{"rendered":"http:\/\/www.safetyphoto.co.uk\/safety_news\/?p=2154"},"modified":"2025-11-17T16:42:28","modified_gmt":"2025-11-17T16:42:28","slug":"manufacturing-company-fined-600k-after-fatal-workplace-incident","status":"publish","type":"post","link":"https:\/\/www.safetyphoto.co.uk\/safety_news\/?p=2154","title":{"rendered":"Manufacturing company fined \u00a3600k after fatal workplace incident"},"content":{"rendered":"<p>Employee died after being trapped under fallen pallet<br \/>\nCompany failed to undertake a suitable and sufficient risk assessment<br \/>\nSystagenix Wound Management Manufacturing Limited t\/a Scapa Healthcare Limited fined \u00a3600,000<\/p>\n<p>A global manufacturing company has been fined \u00a3600,000 after an employee suffered fatal injuries following an incident at the premises in Airedale Mills, Gargrave, Craven.<\/p>\n<p>On 21st September 2020 Tony Snowden, 56, was fatally injured when a \u2018Nelipak\u2019 pallet that was stacked on top of another pallet fell trapping him between the pallet and a ledge on a wall behind him.<\/p>\n<p>The loaded pallet in total weighed 592kg and other employees in the area had to \u2018unload\u2019 the pallet as it was too heavy for them to lift in order to remove it from Mr Snowden. Sadly, when the pallet was removed Mr Snowden had died from his injuries.<\/p>\n<p>A HSE investigation found the company failed to conduct risk assessments considering load, height, weight and stability. The company stored three-legged \u2018Nelipak\u2019 pallets without racking systems, stacking them on top of each other at floor level. The investigation concluded these pallets should never have been stacked vertically.<\/p>\n<p>        RMS area \u2013 where the incident happened<\/p>\n<p>Although the company had a \u201cProcedure for Pallet Handling Policy\u201d not all employees working in the RMS area had been trained on it. No evidence was found that Mr Snowden had been trained on the procedure.<\/p>\n<p>Systagenix Wound Management Manufacturing Limited t\/a Scapa Healthcare Limited of 997 Manchester Road, Ashton Under Lyne, pleaded guilty to a single charge of breaching section 2(1) of the Health and Safety at Work etc. Act 1974.<\/p>\n<p>The court was told that the main failings of the company were:<\/p>\n<p>Failing to undertake a suitable and sufficient risk assessment of the storage of goods pallets in the bulk storage area, and in particular failing to consider their height, weight and stability.<br \/>\nFailing to put in place measures to prevent double stacking and falling pallets;<br \/>\nFailing to provide adequate training for storing goods pallets in the RMS area;<br \/>\nFailing to ensure adequate monitoring and supervision of the condition and storage of goods pallets stored in the RMS area.<\/p>\n<p>The company was fined \u00a3600,000 and ordered to pay \u00a315,000 in costs at Leeds Magistrates\u2019 Court on 12 November 2025.<\/p>\n<p>Further information is available in HSE\u2019s Guidance \u2018Pallet Safety\u2019 \u2013 PM15 <a href=\"https:\/\/www.hse.gov.uk\/pubns\/pm15.htm\">Pallet safety \u2013 HSE<\/a>.<\/p>\n<p>Following the hearing, HSE Inspector Kirsty Storer-Cottrell said:<\/p>\n<p>\u201cThis tragic incident could have been easily prevented had a suitable and sufficient risk assessment taken place and the actions identified implemented.
